What is mesotherapy?

 Mesotherapy or intradermotherapy is a non-surgical procedure, widely used in Europe to treat injuries and medical conditions. It consists of the administration of microinjections with conventional or homeopathic medicines, whose ingredients may vary, but usually include plant extracts, vitamins, minerals, in the middle layer of the skin. Injected substances differ according to purpose and, within the scope of aesthetics, are generally of natural origin.

 

 

Therefore, mesotherapy is considered a relatively invasive technique given the application of intradermal microinjections, at less than 4mm deep of the epidermis.

 

 

Which technique is preferred?

 As for the introduction of the needle into the skin, this varies from author to author, and is described that can be perpendicular or inclined, forming an angle of 30 ° to 60 °. There is, however, agreement among these authors that the needle should penetrate to a maximum depth of 4mm. It is recommended to use the Lebel needle (4mm long bevel).

 The injections should cover only the area to be treated and the distance between them is also variable, being able to be between 1cm (minimum) and 4cm (maximum) between them. The applications reported in the articles are done weekly or monthly and the number of sessions mentioned ranges from four to ten. It is suggested to apply small volumes per puncture.

In addition to the traditional needle and syringe assembly, instruments considered more sophisticated and more expensive can be used: the mesotherapy guns. These guns are multi-point electronic injectors that allow the volume and depth of the application to be quantified. The disadvantage of this system is the difficulty of sterilizing the entire assembly, since only the needle is disposable.

 

 

An important detail in the art ...

The more superficial the injection, the slower the diffusion, the longer the product remains at the desired location. Fifty percent of the amount of drugs injected at less than 4mm depth would remain at the injection site after ten minutes, while 16% of drugs injected at more than 4mm depth would remain in place after ten minutes. It was concluded that the diffusion of a product in intradermotherapy depends on the depth at which it is injected.

 

This distribution difference can be illustrated with elimination curves: the superficial intradermal route would have a monoexponential elimination curve, whereas the deep intradermal route would have a bi-exponential curve (a faster initial elimination, corresponding almost to an injection intravenous, followed by a slower elimination by a dermal reservoir).
