Diabetic Foot Ulcer

Two to ten percent of diabetics have foot ulcers. The risk of developing a diabetic foot ulcer increases with the duration of the diabetes. Unfortunately, most foot and lower leg amputations are performed on patients with diabetes mellitus. The main priority in the treatment of diabetic foot syndrome is to avoid ulceration and amputation.

Diabetic patients and their families must be informed about the correct implementation of measures designed to reduce the risk of diabetic foot ulcers (UPD) and associated complications.

Practical advice for the patient and family

Change your lifestyle to prevent UPD:

  • Make an adequate blood glucose control
  • Control your blood pressure
  • Control your fat intake.
  • quit smoking
  • Practice a physical activity. You should take walks every day.

Advice for skin and nail care to prevent UPD:

  • Wear wide, comfortable shoes with padded support to avoid internal friction
  • Put on seamless cotton socks
  • Wash your feet daily with hot/cold water and a neutral pH soap
  • Inspect your feet daily for signs of injury (heel area, sole  and interdigital area)
  • Dry carefully with small touches (avoid friction)
  • If you have dry skin, calluses or cracks on the skin: apply emollient/moisturizing creams
  • Calluses should preferably be treated by a podiatrist, avoiding the use of keratolytic solutions
  • The nails must be cut straight, it is advisable to use a nail file
  • Never walk barefoot on any type of surface
  • Avoid using heat pads or electric heaters to avoid the risk of burns

Diabetic foot ulcer treatment:

The treatment of diabetic foot ulcers aims to prevent the occurrence of infections and provide an optimized environment for wound healing. If you suspect you are having a diabetic foot ulcer, proper assessment and referral are vital, as a delay in diagnosis and treatment increases morbidity and mortality and can contribute to an increased rate of amputations.

Important factors in caring for patients with diabetes include:

  • Vascular disease control (eg regulation of serum glucose levels and arterial risk factors such as hypertension and dyslipidemia)
  • pressure relief
  • UPD Debridement
  • Treatment of infection: patients with infected wounds require initial treatment with systemic antibiotics.
  • Effective use of wound care products
  • After the UPD has healed, it is important to avoid its recurrence or the development of a new UPD.
