What is mesotherapy?

Mesotherapy or intradermotherapy is a non-surgical procedure, widely used in Europe to treat injuries and medical conditions. It consists of the administration of microinjections with conventional or homeopathic medicines, whose ingredients may vary, but usually include plant extracts, vitamins, minerals, in the middle layer of the skin. Injected substances differ according to purpose and, within the scope of aesthetics, are generally of natural origin.

Therefore, mesotherapy is considered a relatively invasive technique given the application of intradermal microinjections, at less than 4mm deep of the epidermis.

What are the advantages of mesotherapy?

This procedure presents some advantages when compared to other methods, among them:

    •
    • It is a painless method;
    • It is a procedure that does not cause scars;
    • Does not require the use sedatives;
    • You can go home right after the treatment, no need to stay in recovery, as in the case of other more invasive / surgical procedures.

What are the therapeutic indications?

In the early days it was more used in the field of sports medicine. Nowadays, it is used in several areas.

In sports medicine, when there is a soft tissue injury, mesotherapy is one of the techniques that can help sportspeople to return to activity more quickly. The substances used in these cases have anti-inflammatory and muscle relaxant effects.

Mesotherapy can also be useful not in the field of rheumatology, especially when there are tendinites, bursites, osteoartrose, lombalgia, myalgias, and reflexive and reflex muscular contractions. Second or Portuguese Institute of Rheumatology, the secondary effects of these treatments are practically nonexistent.

It can, also, be applied not to treat other medical conditions, such as: fibromyalgia, herpes, scalp, carpal tunnel syndrome, Bell's paralysis, sinusite, insonias, pain and edema, enxaqueca, non-general inflammation, varicose veins, scars , insónia (um dos compostos é a camomila, portanto, são compostos naturais).

A mesotherapy tem ação descontraturante, anti-inflammatory, anti-edematous and analgesic.

Does the Mesotherapy procedure cause pain or discomfort?

In mesotherapy, needles are used so it is normal to feel some discomfort or discomfort in the region of the puncture, not exactly pain. To ease this discomfort, some practitioners apply ice packs or analgesics during the session before the procedure. Ice Spray has an analgesic function that many professionals use for this purpose.

When it is necessary to administer intramuscular application, infrequent situation, anti-inflammatory, relaxing or anesthetic substances are used. The level of pain varies according to the area applied and also according to the sensitivity of each person.

How long will a mesotherapy session last?

The sessions can vary substantially, depending on the type of intervention. However, usually 15 minutes is enough to complete the session properly.

What is the total number of sessions required?

As for the total number of sessions, it also depends on the problem areas to be treated. By month, however, usually 3 sessions of mesotherapy are done.

Who should not do mesotherapy?

contraindicacoes-mesoterapia-mipmed

A technique is contraindicated to:

  • hypertensive.
  • pregnant women.
  • women who are breast-feeding.
  • because the immune system has not yet fully developed.
  • people with skin problems where it is necessary to inject the solution. In this case it is advisable to delay the treatment until this situation is resolved.
  • people who have had cancer for less than a year. They should wait a year for the body to go without receiving chemicals, not because there is any side effect, but because it is recommended to rest the body before doing mesotherapy sessions.
  • people who are prone to severe allergic reactions, in particular, those who are intolerant to the sulphurous acid salt. This substance is used to stabilize the antioxidant substances. Or reactions to parabens. A pre-allergy test can be done by puncturing a small amount in the shoulder region.
  • people suffering from gallbladder disease, heart disease, and autoimmune diseases and neuroses.
  • people suffering from vascular pathologies of nature, such as hypertension or coronary artery disease. It may also include problems with blood clotting.

Who can undergo Mesotherapy?

All persons who do not present any infection in the area to be treated and have not already shown hypersensitivity to the solutions used in the treatment in order to avoid allergic reactions. It is not advisable for pregnant or breastfeeding women.

What professionals can do mesotherapy?

Adequate training is required to practice mesotherapy. Usually there are several members of different health areas that complement their training in this area: masseurs, nurses, physiotherapists, doctors, homeopaths, acupuncturists, pharmacists.

What precautions should be taken before and after each session?

On the day of the session it is not advisable to use any type of cream on the skin, nor should you take any medication containing acetylsalicylic acid (active principle of aspirin, for example) in the days prior to treatment, due to its ability to blood thinner.

Another recommendation is that you do not sauna before the session of mesotherapy, due to the consequent vasodilation that can lead to bleeding occurrence in the application of injections.

After each session it is advisable not to wear very fair clothes and just washed, avoiding acts that could lead to contamination of the places where the injections were applied.

You should avoid doing epilation the next day and should not use body cream for at least 48 hours after treatment.

Within 48 hours after mesotherapy you should avoid:

  • Massage with very hot shower water at procedure site
  • The sun exposure
  • The application of ointments and massage at the treatment site
  • Physiotherapy
  • Hydroginastic

You should apply for 48 hours:

  • Ice with the exception of the spine (to avoid muscular contractures) for 10 minutes at intervals of 2 hours, protected in order to avoid burns. You can use cold compresses or cold spray.

What is the mode of action?

Intradermotherapy has always been described in articles such as the intradermal injection of highly diluted drugs, appropriate for this route of use. The dermis would then become a reservoir from which the products would activate dermal receptors and diffuse slowly, using the microcirculation fluids. The basic procedure of intradermal injections varies greatly from one study to another, reflecting the lack of a methodological standard that supports mesotherapy. In common, such studies describe that mesotherapy consists of intradermal or subcutaneous injections of a drug or a mixture of several products, called mélange.

What is the privileged technique?

As for the introduction of the needle into the skin, this varies from author to author, and is described that can be perpendicular or inclined, forming an angle of 30 ° to 60 °. There is, however, agreement among these authors that the needle should penetrate to a maximum depth of 4mm. It is recommended to use the Lebel needle (4mm long bevel).

agulhas-mesoterapia-mipmed

The injections should cover only the area to be treated and the distance between them is also variable, being able to be between 1cm (minimum) and 4cm (maximum) between them. The applications reported in the articles are done weekly or monthly and the number of sessions mentioned ranges from four to ten. It is suggested to apply small volumes per puncture.

agulhas-mesoterapia-mipmed-material

In addition to the traditional needle and syringe assembly, instruments considered more sophisticated and more expensive can be used: the mesotherapy guns. These guns are multi-point electronic injectors that allow the volume and depth of the application to be quantified. The disadvantage of this system is the difficulty of sterilizing the entire assembly, since only the needle is disposable.

An important detail in the art ...

The more superficial the injection, the slower the diffusion, the longer the product remains at the desired location. Fifty percent of the amount of drugs injected at less than 4mm depth would remain at the injection site after ten minutes, while 16% of drugs injected at more than 4mm depth would remain in place after ten minutes. It was concluded that the diffusion of a product in intradermotherapy depends on the depth at which it is injected.

This distribution difference can be illustrated with elimination curves: the superficial intradermal route would have a monoexponential elimination curve, whereas the deep intradermal route would have a bi-exponential curve (a faster initial elimination, corresponding almost to an injection intravenous, followed by a slower elimination by a dermal reservoir).

What are the advantages anddisadvantages of treatment?

Benefits:

mipmed-vantagens-tratamento-mesoterapia

  • One of the great advantages of this treatment is that it acts very localized, which causes high concentrations of treatment substances to act quickly, thus making the results more visible.
  • Great efficiency.
  • Less painful treatments.
  • There is no risk of anaphylactic reactions.
  • The effect of mesotherapy is guaranteed in the long run.
  • This procedure can be combined with other techniques for correction.
  • This therapy competes with other, more expensive treatments aimed at weight loss or pain management.

Disadvantages:

mipmed-desvantagens-tratamento-mesoterapia

Concerning the disadvantages, it is possible to notice the onset of redness and itching at the site of micro-injections or other minor side effects, as well as the fact that this treatment can not be administered to pregnant or breastfeeding women.

Are there side effects associated with mesotherapy?

Usually not. Although, in general, homeopathic mesotherapy has no side effects, we must not forget that we can find some of these cases, punctual but nonexistent:

  • Pain: can be mechanical, caused by the puncture itself. Sometimes it tries to minimize itself with surface anesthetic products but satisfactory results have not been achieved. It is recommended to use the 30G needle since it is the least traumatic.

dor-mesoterapia-mipmed

If there is more intense pain it can be due to several causes: by an incorrect drug mixture or with a non-physiological Ph, vasoconstricting products or by a poorly performed technique (which no therapist will recognize). There is no justification in Mesotherapy for a drug to produce pain if we comply with the proper criteria of use.

  • ERITEMA: We can find very common erythema produced by the simple act of puncture itself, in extremely sensitive people, also with skin necrosis usually produced by deepening too much with the needle, going through very serious allergic reactions, which with homeopathic products. it is virtually impossible for them to occur. Do not open an ampoule and leave it open for a day or two for use with another patient. Such a procedure may cause an allergic reaction not to the product itself but to the contamination it has undergone.
  •
  • Mycobacterial INFECTION (very rare) that requires treatment. These infections can result in unsightly scarring. This secondary infection is explained by improper pre-procedure asepsis or contamination of the product used.
  •
  • VAGAL REACTIONS: What in common sense is called fainting. It may occur in the case of very nervous or anxious patients. Only the sight of the needle can cause them to sweat and dizziness, so it is important to always have sessions with the patient leaning.

  • NERVE AND VESSEL INJURY: It can be due to the anatomical proximity or when we perform the treatment too quickly or by reflex movements of the nervous patient. They are characteristic of temporal, epitroclear and peri-varicose zones.

  • HEMATOMAS: Very common in mesotherapy, if the cause is the technique means that the puncture is too deep. They are not negligible however small, some could have very serious effects as they are the ideal medium for growing many germs. No medication should cause bruising on mesotherapy.

  • CUTANEOUS NECROSIS: It is, along with allergic shock, the most feared side effect in mesotherapy. Its origin may be chemical (very dense products, some non-steroidal anti-inflammatory drugs, over-the-counter cocktails, mucopolysaccharidoses) or of biological origin. The best way to avoid it is with the right technique, proper asepsis and skin disinfection, and the use of drugs that meet the criteria for use for this route

  • GENERAL FATIGUE.

  • Other, even rarer complications reported are: lichenoid eruption, induction of psoriasis, urticaria, skin necrosis, systemic lupus erythematosus, panniculitis, acromy, and atrophy. Such complications are attributed to the use of improper technique or the effect of the drug itself.

Does Mesotherapy Really Work?

This is a topic that has generated many doubts and discussions. In fact, there are several published studies on clinical trials and treatments, as well as on the effects of these medicines and the numerous ingredients injections can use. In some situations, there is no hard evidence to support their action. For example, in the particular case of fat loss, there are not many studies on whether mesotherapy really targets fat cells (adipose), yet the evidence speaks for itself. Many people who have already resorted to this procedure talk about its benefits and that it really works. Mesotherapy is an increasingly used method. Before resorting to it you should be properly informed and take care in choosing the clinic and the professionals who should be properly certified for this purpose.

What if we told the historical review that underlies mesotherapy?

The history that drove the intradermal therapy technique is well known. Pistor received a patient with an asthma attack and administered intravenous procaine for bronchodilation. This patient also had a chronic hearing deficit. The next day, the patient reported to the doctor that after 40 years of deafness, he had been able to hear the church bell again, relating it to the injection received. Hearing had improved briefly. The doctor began to give intradermal injections of this product in the mastoid region and the patient presented temporary recovery of hearing. Physician proceeded with procaine injections in several patients and, in 1958, published his findings in La Presse Medicale, in an article entitled: "Exposé sommaire des propriétés nouvelles de la procaine local en pathologie humain" (Brief Exposure of New Properties) of locally applied procaine in human pathology).

The doctor concluded that sensorineural stimulation was produced by procaine, but in a short period. Pistor believed that this therapeutic modality based on intradermal injections was so important that it deserved its own name, mesotherapy, given the embryological origin of the dermis.

Although this is the best known historical landmark of intradermotherapy, there are previous experiences that supported Pistor's conduct. Homeopathy was first mentioned by Hippocrates (462-377 BC), but it was physician Samuel Hahnemann (1755–1843) who laid down the basic principles of homeopathy: law of similarity, healing direction, principle of the single remedy, the theory. minimum diluted dose and chronic disease. In 1884, Koller, an ophthalmologist, reported his experience using local cocaine for pain management. In 1904, Einhorn discovered a new anesthetic with a lower risk of triggering addiction, procaine. In 1925, Leriche applied intradermal injections into the intercostal spaces. In 1937, Aron published a study on intradermal injection of a histamine solution and concluded that intradermal injection of any product at a painful site would have an analgesic effect. However, it was from Pistor that intradermotherapy received the most attention, and the French Society of Mesotherapy was founded in 1964. Thus, the technique spread throughout the world.

Pistor, in 1976, summarized the technique with the words: "Little, few times, and in the right place." The founder of mesotherapy himself acknowledged that such recommendations were empirical and based on his personal clinical experience. He said he realized that while larger doses made no difference to the clinical outcome, multiple punctures appeared to be better than fewer injections.
